For safety reasons, the health office cannot store medications for your student over the summer.  Please make arrangements to pick up medications at the end of the school year.  Medication not picked up within 5 days after dismissal of the 15 – 16 school year will be discarded.

A medication authorization form for medication for the 2022 - 2023 school year must be completed by physician and signed by parent after June 30, 2022.  Please make arrangements to get the authorizations completed over the summer.  Student medication and authorizations may be brought to school on the first day.  The link to the medication authorization form can be found below.  All medications dispensed at school, even over the counter medications, need to have a medication authorization form.

For your student’s safety, please update the school nurse with any new medical conditions or medical needs that your student may have.  Medical conditions and medical diagnosis can be updated with a letter or note from the student’s physician.  If there is a new medication or a change in medication needs for school, please complete the medication authorization form.
